This article will explain how to create a questionnaire on the Human Risks platform.

To create a new questionnaire, go to the Questionnaires module and click on “Create +” found in “Your Questionnaire(s)” section.

  1. You can select to copy an existing questionnaire, so you don’t have to start from scratch.

  2. Choose the division(s) the questionnaire should be part of. You can add an individual who might not have access to the selected division, by clicking on the user icon.

  3. Write a title.

  4. Add a description if relevant.

  5. Click on save, to start building the questionnaire.


Start building your questionnaire

The previously added information "Title" and "Description", can still be edited, you will find their fields at the top of the page.

In order to create a new question, you will have to scroll to the bottom of the “The Questions” list and press on the “Add question”. Questions can be organised in groups for a better organization. To create a group use the “Add group” button and define a title for your group.

Create a new question

You can select between the following types of questions:

  • Slider - allows the user to select a value within a given range by pulling the markers along the horizontal track.

  • Text – allows the user to answer with a text response.

  • Checkboxes – allows the user to select one or more options from a set of multiple options.

  • Radio buttons - allows the user to choose between either one or the other answer. Which gives you the ability to make questions where the respondent can only choose one answer out of the different options provided.

  • Number – allows the user to answer a number within a range defined by you.

  • Location – allows the user to select a specific location from the world. The user can type the address. If you have access to external intelligence providers, you can define how many points a question should score based on the risk ratings from the external intelligence provider.

To exclude the question from being included in the calculation of the score please check the box right next to the “Score”.

If you want to make the question mandatory for the respondent to answer then check the “Required” box.


Score required

You can define the criteria for how the questionnaire gets scored by using the sliders at top of the page.

Depending on how many points your entire questionnaire consists of you will be able to define the scoring of the questionnaire.

The slider consists of numbers from 1-100 which represent the percentage of the total amount of points that your questionnaire constitutes.

With the slider, you can decide how many percentages of the total percentage should represent the colors red, amber and green.

Each question can have an x amount of points allocated. When creating a new question, you can define how many points the question should have when answered. You can also modify the amount of points by editing the question. Some questions types can have multiple options on how the points are calculated. For example, on Checkbox type you can define the points system to be either:

  • Max - Provide only the highest selected value

  • Accumulated - Accumulate all points for the selected options


Question functionalities

Right next to each question there are multiple specific functionalities that can be performed on the question. You can edit, delete or copy the question. If you want to change the order of the questions, you can do it by dragging and moving up or down on the list of your questions.

Logic

You can also add logic to your questions by clicking on the logic icon. This can only be added to the second or the following questions. The feature lets you define if the question should be shown to the user depending on how the user responded to one or several previous questions. Note that the questions that are not shown to the user are not included in the score.

Issues

Additionally, you can click on the exclamation mark to make certain answers raise an issue. By clicking on “add logic” you can choose which question and answer should trigger an issue. Next, define the issue by choosing a title, category, and severity level of the issue. You can also add a description of the issue.

In cases where an issue is raised, you can assign a corrective action to the issue. A corrective action is a task that can be assigned to the person responsible for correcting the issue raised.


Options

Each answer option can result in a number of points. You can define the colour of the box around the option when creating or editing the question.

Additionally, you can edit, delete and move each options.
