To create a new incident report please follow the steps below:
Go to your platform
Go to the Incident reports module.
Click on Create+
Choose between either creating a new incident report from scratch or by using one of the templates created on the platform.
Start filling out the information required. Note, if you have Pattern Recognition enabled, it might look different. For more information check out this article.
Division - Select which division(s) this incident report should be related to. Only users with access to that / those division(s) will be able to see the incident report.
Incident Category & Type - Select the most relevant incident category and select the most relevant incident type from that list. Each category holds different types. If you can't find a suitable category or type please select “Other” and describe the incident in the “Description” field.
Start & End Date - Indicate when the incident started and ended.
Near Miss - If the incident was an unsuccessful attempt i.e. an attempted burglary you should select “Yes”.
Loss - Users with the role "Manager" or "Administrator" can apply a loss to the report. If applicable indicate the financial loss as a result of the incident.
Description - Describe the incident. Please make sure your language is suitable for sharing. Keep in mind this document is for official use. Please refrain from using informal language. Remember not to include any personal data such as name, ID-no., e-mail address, telephone no., or any other data that could connect to a person.
Risk Assessments - Here you can select a number of Risk Assessments to link this incident report to.
Sev. level - Indicate the severity level of the incident report.
Location - You must input your own location unless your browser is allowed to detect it automatically. The map is linked to Google Maps and will identify the city and country when you search for the location. If the exact location isn't found, you can use the location icon on the map to pinpoint it accurately.
When you are ready to submit your incident report, please click on the “Create” button. Note, all required fields need to be populated in order to create.
Once you have created the report, you can attach files such as any relevant documentation i.e. photos, documentation of reporting to local authorities, receipts, or similar.
Please be aware that there is a file size limit of 50 MB per file for uploads in Incident Reports!
You can also add a task with any follow up to do's related to your incident. For more information on how to create a task, read this article. Lastly, you can also add comments to the incident, and tag users from the platform by using the @ symbol.
