Create a new custom form

This articles explain how to create a custom form on the Human Risks platform.

As an administrator on the Human Risks platform, you can create custom form templates that you want to be used when creating a new report in Incident Reports module. This can be done on the Form Builder page. On the main page of the Form Builder module, you will find a list with all the Forms previously created. On the list, you can also find the type, the date and the user which created it. Additionally if you want to edit or delete a form, you can do it by using the symbols from the right side of the page.

If you are looking for a specific form, you can use the search bar to search it by title or author.

How to create a new Custom form:

  1. Go to Form Builder

  2. Click on "Create +" button

  3. Choose "Create your own"

4. Give a name to your Custom form.

5. Drag and drop a content field from the left side menu into the center of the page. You have also the option to make a content field required or apply a "Display Logic" to it.

6. After you are done creating you form click on Save button found right next to the title.

Content Fields

You can use different types of content fields to customize your form:

  • Text - Allows the end-user to add a small text

  • Text Area - Can be used in scenarios where you want to provide the end-user with the option to type a more detailed text

  • Number - Allows the end-user to select a number between two pre-defined one

  • Location – Allows the end-user to select a location

  • Dropdown – Provides the end-user with a drop-down list

  • Date - Allows the end-user to select a date

Display logic

Display Logic lets you define if the option should be shown to the user depending on how the user responded to one or several previous options.

Below is an example of how to fill out a form using display logic and various content fields:

Additionally, you have the option to organize multiple content fields into custom form headings, grouping them together and specifying the name for each group.

For instance, in the example below, the custom fields "Text Area" "Dropdown" and "Text" are grouped together under a custom form heading labeled "Group 1".
