This article explains how to set up the Site Category Questionnaire

As an administrator you can set up a questionnaire that will calculate the site category for the site you are trying to create a Risk Assessment for.

  1. To set it up, go to the platform and click on Organization.

  2. Scroll down to the Site Category Questionnaire part of the page.

  3. Click on Edit: Site Category

  4. Now, you can start creating the questions for your questionnaire.

Each question can be matched with different types of options/answers:

  • Slider

Sliders allow you to answer on a range from X to Y. You can define X and Y yourself and you also can choose the maximum value and how much each step should increase by.

  • Text

The text option allows you to create a question where the respondent needs to fill out a text answer. Additionally, you can select what type of text the respondent should write. You can choose free text, custom regex, email, and phone number. Customer regex is a specific pattern type that the answer should follow.

  • Checkboxes

Check boxes allow the respondent to check one or multiple answers.

  • Radio buttons

The Radio button option lets you choose between either one or the other answer. Which gives you the ability to make questions where the respondent can only choose one answer out of the different options provided.

  • Number

The Number button allows you to select a minimum and a maximum value which allows the respondent to select a number within that range.

  • Location

The location option allows you to add a question where you need to provide a location as the answer.

Each option can also give points if you check the points box.

  1. Define how many points each option should count, this is a crucial part of making the site category questionnaire.

  2. Once you have added all the questions and the respective options, you will get an overall score.

  3. Click into each site category to indicate the percentage each site category should weigh in the site category classification based on the overall score.

    Example

E.g. you have 1000 as an overall score and have defined the weight of the site categories as seen above. Your response to the questionnaire consist of a score of 600 points, then in this case your site category classification will be Medium, because it is equivalent to 60%.
